It could be a ruptured disk in your neck. Or it could be referred pain from your lungs from a tumor, cancer, or inflammation of your lung ’sacs’. You need to see a doctor.
And stop smoking unless you don’t care if you get cancer and die an early death. And even if you don’t care, I’m betting your family will miss you if you die too young.

Could be a pulled muscle or spinal disk injury. You would rather have the pulled muscle :-)
Oftentimes smokers cough more than usual and that pulls muscles.
    Consider evaluation by a chiropractor.

I would suggest going to the doctor to get checked out. My dad had pain in his shoulder so bad he rubbed it raw. Turns out, he was going to and did have a massive coronary heart attack and is now mentally disabled.
Don’t wait… even if it’s something small, just get it checked out!

Although it could be a bad problem and should be checked out by an MD, I will give you another explanation for what it might be.
There are a group of muscles between your shoulder blades that are called the Rhomboids. They could be extremely tight and that is what is causing you pain. This is a common problem, especially if you work with your upper body all day (typing included). Also, the pain when you look down to the left or the right sounds like a muscle called the Levator which attaches under both shoulder blades and then at the bone behind your ear. This could have shortened up from some repetitive motion that you are doing above your head, or by the position that you sleep in.
I would recommend making an appointment for a therapeutic massage so that muscles can be assessed, as well as seeing your dr. In the meantime, try moist heat around your neck and shoulders and see if there is any improvement in your pain. If so, it is most likely muscular.
    Good luck! :)
