If you’ve had no direct trauma to the area you can probably rule out a fracture. Sounds to me like a possible rotator cuff tear. The “rotator cuff” is actually four muscles that function to move as well as stabilize the shoulder. The most common of these muscles to be torn is the supraspinatus. This would explain the pain while lying on your side. Try standing with your arms at your side. Now lift the arm that hurts out to your side as far as you can. Are you able to get past 90 degrees without any pain? If not, you may have a tear or other disfunction…best to see the doc for an evaluation and treatment.

Maybe you should get it x-rayed. You could have a torn rotator cuff, or a bit of broken off cartiledge. Definitely get it checked. Hopefully, it’s just a muscle issue and a chiropractor can fix you up in a few visits.
See a doctor immediately. You may have lung or heart problems or a fractured rib.
